Strong Answer Example:
Context: "At Microsoft, I accidentally deployed a configuration change that caused our login service to fail for about 15,000 users during peak hours. I was working on improving our deployment process and didn't properly test the configuration change in our staging environment first."
Immediate Response: "When I realized what happened, I immediately took responsibility and worked to fix the issue: I quickly rolled back the configuration change; I communicated with my team lead about the situation; I started a user impact analysis to understand exactly what went wrong; I documented the exact steps that led to the failure so we could prevent it in the future."
